When designing your Pretender, the form you go with is the "chassis" that you stuff your Design Points into, adding Dominion strength, Scales, and Magic paths for your Bless. What you choose matters more than you probably think, since even Imprisoned pretenders can handle only so much. Your Pretender isn't just a figurehead, either, as they have the capability to remain your most useful unit well into your game.
Your Dominion strength is determined in Pretender design. This determines BOTH how many of your Dominion's candles can be in a province (aside from Preaching or events) and how well it holds up against rival Dominions. Having more candles in a province also adjusts the local Scales faster. While you can increase Dominion strength with 5 Temples per candle, so can everyone else, and it's better to start with the advantage.
Different forms have a different starting Dominion strength, falling into four general categories that happen to coincide with other traits.
Dominion 1 forms are generally Mages, "Gods among Men", a bit like superheroes… Iron-Man-or-Doctor-Strange-style superheroes.
Dominion 2 forms are generally Monsters, worshipped only so that they don't eat their worshippers, which (often) works now!
Dominion 3 forms are generally Titans, powerful forces with enough humanity (physical and otherwise) for mortals to connect with them.
Dominion 4 forms are generally Idols, representations of and/or houses for ascendant spirits, sometimes powered by faith alone.
These generalizations aren't absolute, of course, but the furthest deviation you'll see from them is by 1 candle; for example, ascended Angels are typically Dominion 2 forms, despite looking and feeling more like (depending on the species) either Titans or Mages.
Each upgrade to your Pretender's Dominion strength costs (7*n) Points, with n being the number of upgrades. Buying one candle costs 7 points, buying a second candle costs 14 points, buying a third candle costs 21 points, and so on. Your Mages and Monsters are actually more expensive than they appear, at least if you plan on starting with a specific Dominion strength.
Looking at the pictures above, you can hopefully deduce that Monsters bring the most physical power, followed by Titans, while Idols are hard to destroy. You can't quite improve a Pretender's physical power directly; all of that comes from Indirect Magic bonuses from having Paths (such as additional Natural Protection from having Earth Magic) and your Magic-level-built Bless. So what about magical power?
Most forms start with meager Magic levels, but Design Points that remain after picking your form can be spent for "level-ups". Each upgrade to a Magic path costs (8*n) Points, with n being the number of upgrades in that path. The exception is buying the first level if you don't already have a level in that path, which has a fixed cost that varies with the Pretender. For example, a red-robed Arch Mage starts with 1 and has a New Path Cost of 10, so getting him to 4 costs 48 (8+16+24) Points, while getting him to 4 costs 82 (10+16+24+32) Points.
Mages are the cheapest forms, followed by most Idols, and then followed by most Titans and Monsters. The Mages are as close to a blank canvas as you can get for building your preferred Magic & Scale combination, while other forms come with significant costs for going "wide" in your paths. On the other hand, if you want to go deep into a specific path (perhaps for a strong incarnate Bless), then you should look for whichever form has the highest level in that path at the lowest cost. This typically won't be a Mage, since Mages are such a blank slate that their starting Magic levels are typically 1 in a path or two.

Most Pretender forms are regional or national – they might be a cultural thing, or they might be something explicitly tied to the faction's history. Only one form is shared by absolutely every nation in the game, but a few others are so widely available that a nation not having them is noteworthy. As nations and races interact through the Ages, the pool of Pretender forms that each nation will accept typically widens.
Underwater nations don't allow Pretender forms that aren't inherently Aquatic or Amphibious, so their selections are almost-entirely regional. Mekone & both Phlegras don't accept Mages due to the Gigantes' fervent hatred for "the Gods of Man" and other posers, while Ind doesn't accept Dominion 1 forms because their standards are too high.

The Deeps is the name provided for the underwater region in Dominions 5's Modding Manual. Any bleed-over from this region to others is typically from nations being driven on-land, or from cuddly Amphibious Titans such as the Drakaina straddling two regions with their massive tails; these region-straddling forms are often more common in their secondary region than in this region, since the eleven factions under the sea aren't exactly uniform:
